An Interview With Dr. Majid Fotuhi, Author of The Invincible Brain

Your newest book refers to the brain as being “invincible” when treated properly. What does that mean to you, and what are most people misunderstanding about brain aging?
When I say the brain can be invincible, I mean it is far more adaptable and resilient than most people realize. Your brain is not a fixed structure like your ears; it changes in response to how you live.
The biggest misunderstanding is that memory loss is inevitable and that Alzheimer’s disease is purely genetic. In reality, daily habits shape brain structure and function across decades.
Your research suggests cognitive decline can be prevented and even reversed in as little as 12 weeks. What makes your Brain Fitness Program different from typical “brain health” advice?
Most advice about brain health is general and fragmented. My program is structured, measurable, and multimodal. It combines exercise, nutrition, sleep optimization, stress regulation, and targeted cognitive training in a coordinated way. When these pillars are implemented together, we can see measurable changes in memory scores and even brain imaging within weeks, not years.
More than 80% of my patients who completed my 12-week program, even elderly in their 70s, had remarkable improvements in their standard cognitive tests.
One of the most compelling ideas in your book is the link between brain health and sexual health. Why are libido and cognition so closely linked?
Both depend on healthy blood flow, balanced hormones, and intact neural circuits. The same vascular system that delivers oxygen to the hippocampus also supports erectile function. Testosterone, dopamine, and nitric oxide influence motivation, mood, and desire. When men improve metabolic and vascular health, they often see gains in both mental sharpness and sexual vitality.
When talking with my patients who are men, I always remind them that the very factors that boost brain performance also elevate sexual performance, and so they have multiple reasons for following my brain health recommendations!
For men in their 40s, 50s, and 60s experiencing both mental fog and declining libido, where should they start?
Start with movement. Aerobic exercise improves cerebral blood flow, boosts testosterone modestly, enhances insulin sensitivity, and increases growth factors that support brain cells. Then prioritize sleep. Deep sleep regulates hormones and clears metabolic waste from the brain. Those two steps alone often create noticeable momentum.
What does a real “midlife upgrade” look like in practical terms? If someone wants better focus and better performance in the bedroom, what are the first three habits to fix?
First, exercise at least five days a week with a mix of aerobic training and strength work.
Second, eliminate ultra-processed foods and adopt a Mediterranean-style diet rich in olive oil, vegetables, fish, and nuts. Third, make sure you get seven to eight hours of high-quality sleep.
These three habits improve vascular health, hormone balance, and neuroplasticity simultaneously.
What are some overlooked daily habits that quietly sabotage both mental sharpness and sexual vitality?
- Chronic sleep restriction.
- Excess alcohol.
- Sedentary workdays without movement breaks.
- Constant digital stimulation late at night.
- And unmanaged stress that keeps cortisol elevated.
Each of these erodes blood vessel function and disrupts hormonal balance over time. Each of these factors is associated with shrinkage in brain areas that are critical for learning and memory.
After more than 35 years of neuroscience research, what discovery surprised you the most?
What has surprised me the most is how quickly the brain can respond when multiple lifestyle factors are optimized together. I have seen thousands of individuals in midlife improve measurable memory performance in a matter of weeks. The brain is dynamic, even in our 60s and 70s.
What’s one myth about brain aging you wish people would stop believing?
That cognitive decline is just “normal aging,” and nothing can be done about it. That belief often becomes a self-fulfilling prophecy. You really can keep your brain sharp as you age.
What motivated you to write The Invincible Brain at this point in your career?
One of the most rewarding parts of my job as a neurologist has been seeing patients who come to see me after they completed my 12-week program and express how amazed they are with the degree of the boost in their memory, attention, processing speed, and problem-solving. They often cannot believe how much they have improved in such a short time. Their happy faces, their success stories, and their expressions of gratitude motivated me to write my book.
